Step 1 - As early as possible, but no later than three weeks before any instruction begins, contact:

UCSB Extension (Attn. Disability Services Representative)
    Mail Code 1110
    University of California, Santa Barbara
    Santa Barbara, CA 93106-1110

    Hours: 8am-12noon and 2-5pm, Monday-Friday

*You may also use the California Relay Service (CRS) by calling (800) 735-2929 TDD or Voice; or (800) 735-2922 Voice to TDD (relays telephone calls between TDD and voice callers).

At this time, the representative will discuss your individual needs and will set up an appointment to meet with you during the next seven to ten days.

Requests to meet with the representative received later than two weeks before instruction begins may limit our ability to provide you with the requested services in time for the first day of class.

Step 2 - Send the representative the following items within the next seven to ten days or in time for the meeting:
    • A written request outlining your needs and describing the limitations you have because of a disability covered under the Americans with Disabilities Act of 1990 or Section 504 of the Rehabilitation Act of 1973.
    • Written notification on official letterhead from a physician, certified specialist, professional counselor, or appropriate school official verifying your disability and outlining any need for special services. Depending on your needs, additional documentation may be requested.

Step 3 - Meet with the representative. At the meeting, your request will be confirmed and a plan will be determined for providing you with services including establishing a time line for when services will begin. If you have not already submitted the items requested in Step 2, please bring them with you to meeting. Services will only be arranged once you provide payment in full for UCSB Extension course(s)*
